This form functions as a ledger for the league to monitor the payment of league fees for individual players. Only list the names of the players for whom you are currently submitting payment. Submit a new form if you are paying for additional players at a later date. Each team is eligible for up to 3 scholarships if their roster exceeds 22 players. For instance, if your team has 24 players, you may apply 2 scholarships for the 24th and 25th players.
